PianoForKids.com Release Form

Thank you for your artwork or your story! This release gives us permission to place a representation of your artwork or story on the PianoForKids.com Website or on any other PianoForKids.com publication.

There are two ways to submit material:

  • By e-mail. We can accept pictures submitted electronically in either GIF, JPEG, PICT or BMP format. Many mail programs allow you to attach a picture file.

  • By regular mail. Send your pictures to the following address and we will scan them in for you.

If you are willing to have your artwork or story displayed on the PianoForKids.com website, download this form to your desktop as RTF (rich text format), open it with any word processing program, fill it out, and email it as an attachment with your pictures and stories.

Or mail a printed copy along with your artwork and story.

I agree that the photographs, artwork, audio, video, or writing submitted may be used by PianoForKids.com, its assigns or successors, in whatever way they desire, including the Internet, television, CD-ROMs, and any other form for the storage, retrieval and reproduction of information, images; furthermore, I hereby consent that such information, photographs, videos, and the plates and/or tapes from which they are made shall be their property, and they shall have the right to sell, duplicate, reproduce and make other uses of such information, photographs, videos, recordings, and plates as they may desire free and clear of any claim whatsoever on my part.
